If you have scheduled your first appointment with A New Day Hypnosis, please look below to find your appropriate new client forms. We encourage you to print our your forms, fill them out, and bring them with you to your first session. This will allow you to take full advantage of your appointment time.

To ensure a positive and respectful experience for all clientele and staff, we remind you that A New Day Hypnosis requires 24 hours notice for any missed appointment. If the client fails to inform A New Day Hypnosis staff of their absence within 24 hours of their scheduled appointment, the client will be charged 50.00/hr. for their missed appointment.
